All other covered services are subject to a $250 deductible.
After the deductible has been met, services are covered at 85% for the remainder of the plan year.
After the $1750 maximum out-of-pocket has been reached, services are covered at 100% for the remainder of the plan year.
The plan year begins on September 1 and ends August 31.
