Premiums are based on the participant’s age at the time of enrollment.
Employees and their spouse’s premiums are withheld from the employee’s payroll check. All other persons eligible to participate will be direct billed.

All applications will be processed by C.N.A.
Employees can submit their applications electronically using UT Touch Online.
