Evidence of Insurability Requirements

• Add a spouse or dependent(s) to your medical plan

• Increase your life insurance coverage

• Increase your spouse’s life insurance coverage up to $50,000

• Enroll in the Long Term Disability Plan

• Add a spouse or other eligible family members to the Long Term Care Plan

Evidence of Insurability is required during Open Enrollment if you plan to make one of the following changes to your benefits:
