Evidence of Insurability Requirements

• Proof of other group coverage is presented at the time of application. Spouse’s or dependent’s employer plan, Medicaid, CHIP program is accepted as other group coverage.

Acceptable forms of proof include:

• Paycheck stub indicating a deduction for health coverage

• Letter from plan administrator or employer

• HIPAA Certificate

• Enrollment Form
