Get Ready for Your Next Doctor Visit at Merckmedco.com

DoctorTrying to remember what prescription-related items you want to talk to your physician about during your office visit can sometimes be a challenge.  Merckmedco.com, the official web site of your prescription drug benefit program, can help make it easier.

The resources and tools available at “My Doctor Visit” at merckmedco.com can help you save time by consolidating your (and your covered dependents) prescription history and benefit information into one convenient place for you to review, print and share with your physician.  The information that can be found at “My Doctor Visit” includes:

MEDICATION HISTORY
The Medication history section of “My Doctor Visit” lists all of the prescriptions filled by your plan, including prescriptions filled through your home delivery pharmacy, as well as prescriptions filled at a retail, or walk-in, pharmacy.  Medication history lists each prescription number; the name, quantity and strength of the medication; the date prescription was filled; the days’ supply – which refers to the number of days of medications prescribed.

PATIENT REQUESTS
The Patient Requests section list any prescription-related requests you have made over the past seven days.  These include prescription renewal requests, new prescription requests and requests to take advantage of your home delivery pharmacy benefit.

BENEFIT INFORMATION
The Benefit Information section reflects your prescription plan’s copay information for brand and generic medication.

FORMULARY QUICK GUIDE
The Formulary Quick Guide lists most of the commonly prescribed medications preferred by your prescription plan.  In addition, the quick guide lists the lowest and highest relative cost for a medication and whether or not a generic brand is available.

PRESCRIPTION FAX FORM
You can print and bring a prescription authorization form to your health care provider to request new prescriptions.  Your doctor can fax the completed form, which should include an attached copy of the prescription, to the Merck-Medco home delivery pharmacy.  The prescription must include the patient’s full name and date of birth, the date the prescription was written and the prescribing physician’s signature.  A fax number is included on the form.
