In-Network: 85% after $250 deductible
Out-of-Network: 65% after $500 deductible
Out-of-Area: 80% after $250 deductible
Diabetic Supplies are covered by your prescription
drug plan, Merck-Medco Managed Care.
Covered Services:
- Test strips for blood glucose monitors
- Visual reading and urine test strips
- Lancets and lancet devices
- Insulin analogs
- Injection aids
- Syringes
- Non Prescriptive oral agents for controlling blood sugar levels
- Glucagon emergency kits
Benefits:
- One applicable copayment for all supplies ordered with insulin
- One applicable copayment for each item ordered without insulin
- Insulin and oral agents are subject to separate applicable
copayments
- For a 30-day supply of your medication at the retail or local
pharmacy, your copayment is $10 per generic drug, $25 per preferred
brand name drug, and $40 non-preferred brand name drug.
- For up to a 90-day supply of your medication at the home delivery
pharmacy or mail service pharmacy, your copayment is $20 per generic
drug, $40 per preferred brand name drug, and $60 per non-preferred
brand name drug.

Informational meetings on the Texas Tomorrow Fund
will be held on Wednesday, January 16th from 10:00 a.m.-12:00 p.m. and
2:00-4:00 p.m. at the Benito Longoria Conference Room in the Oliveira
Library. The Texas Tomorrow Fund is a pre-paid college tuition
program that allows you to pay for your children’s college education
at today’s prices. A representative from the State
Comptroller’s Office will be present to answer questions about
eligibility, contract prices, payment options and the enrollment
process. The current Enrollment Period ends on May 24, 2002.
