What is Evidence of Insurability?

Evidence of Insurability is documentation of a person’s health history used by insurance companies to determine whether a person meets the definition of good health. Evidence of Insurability is required during the Open Enrollment Period if you plan to make one of the following changes to your benefits: 

  • Add Long Term Disability (LTD) coverage

  • Increase life insurance coverage

  • Add dependents to medical plan

Evidence of Insurability requirements are WAIVED if you plan to add a dependent to your medical plan as long as: 

  • Proof of other group coverage is presented at the time of application.  Medicaid is accepted as proof of other group coverage.

  • Your dependent is covered by the other group coverage at the time of application.  Acceptable forms of proof include:

  1. Paycheck stub indicating a deduction for health coverage

  2. Letter from plan administrator

  3. HIPAA Certificate

Evidence of Insurability forms are now available at Human Resources or online at www.utbtsc.edu/human_resources .  If you plan to make any of the above changes to your benefits, please complete and return your application to Human Resources prior to the Open Enrollment Period or at the latest by July 31st.  Approval from the insurance company is required before these changes can be made to your benefits.

Tuition Assistance for Summer Session I

Planning to take a class this summer?  Apply for Tuition Assistance at Human Resources from May 6-13.

Regular full-time employees who have completed their six-month probationary period are eligible to apply for Tuition Assistance for Summer Session I starting Monday, May 6 at 8:00 a.m. through Monday, May 13 at 5:00 p.m.  Applications will be considered on a first-come, first-serve basis while funding is available.  Interested employees should apply as soon as possible but are strongly encouraged to enroll for classes by telephone or on-line registration BEFORE submitting an application form to make sure the requested class is available.

Application forms are available in Human Resources or online at www.utbtsc.edu/human_resources/.  Only revised application forms dated February 2002 will be accepted.  For more information, please contact Lynne at extension 6530.

  • Recognize and assess emergencies

  • Evaluate victims to provide necessary care

  • Perform rescue breathing

  • Execute chest compressions (adults only)

  • Provide care to choking victims
